If you are over 50, there are two numbers worth knowing. One can be checked with a tape measure. The other needs a blood test.
In a new study, adults with a waist above the abdominal-obesity threshold and severely low vitamin D had a 123% higher adjusted mortality rate over six years than people with neither condition.
That is the alarming bit. The less alarming bit is that this was an observational study. It found a strong association, but did not prove that belly fat and low vitamin D caused the deaths. Nor did it show that swallowing vitamin D tablets cancels out the risks of carrying too much weight around your middle.

The Study: How it Worked
Researchers analysed 5,520 men and women aged 50 and older from the English Longitudinal Study of Ageing. Their average age was 66, and they were followed for six years.
At the start, researchers measured each participant’s waist and blood concentration of 25-hydroxyvitamin D. For men, abdominal obesity was defined as a waist greater than 102 cm. Vitamin D deficiency meant a reading of 30 nmol/L or lower.
During the follow-up, 419 participants died.
Compared with people who had neither condition, those with abdominal obesity but sufficient vitamin D had a 47% higher adjusted mortality rate. Those with vitamin D deficiency but no abdominal obesity had an 81% higher rate.
People with both had the highest result: a 123% increase.
The researchers adjusted for age, sex, smoking, alcohol, physical activity, diabetes, cancer, heart and lung disease, BMI, vitamin D supplementation and numerous other factors.
The study was published in Diabetes, Obesity and Metabolism.
What “123% higher” Actually Means
It does not mean 123 out of every 100 people died, despite what your social-media feed may shortly suggest.
It means the rate at which deaths occurred during the study was 2.23 times higher in the combined group than in the group with a smaller waist and sufficient vitamin D. That is a relative comparison after statistical adjustment, not anyone’s personal chance of dying.
There was also an awkward wrinkle. Among people without abdominal obesity, vitamin D insufficiency was associated with a 91% higher mortality rate, while the more severe deficiency category was associated with an 81% increase.
In other words, there was no neat pattern in which risk climbed steadily as vitamin D fell. That does not make the finding meaningless, but it should discourage anyone from presenting the study as proof of a simple cause-and-effect relationship.
Flab and Low D Partners in Crime
Vitamin D is fat-soluble, and people with obesity commonly have lower concentrations circulating in their blood. Some vitamin D may be stored in fat tissue, while obesity may also affect the enzymes involved in processing it.
Visceral fat is independently associated with inflammation, insulin resistance and metabolic disease. Low vitamin D may therefore be part of the same unhealthy picture, rather than the lone villain responsible for it.
Low vitamin D can also be a marker of poor health, inactivity, limited time outdoors or frailty. The study measured waist circumference and vitamin D once at the beginning, so it could not track how either changed over the following six years.
The researchers also used waist measurements rather than scans of visceral fat, and almost 40% of the original study sample was excluded because measurements or other information were unavailable.
Getting Waisted
Australian guidance says most adult men face increased chronic-disease risk once their waist reaches 94 cm, with risk becoming substantially greater at 102 cm.
Measure halfway between the bottom of your ribs and the top of your hip bones. Breathe out normally, keep the tape snug without pulling it tight, and resist the ancient male tradition of sucking everything in.
The thresholds can differ for men from some ethnic backgrounds, but the broad lesson holds: waist circumference can reveal metabolic risk that body weight alone may miss. The RACGP recommends considering waist circumference alongside BMI.
You do not need to achieve a magazine-cover waist to improve your health. Losing around 5% of your body weight can produce meaningful benefits, while regular resistance training, walking and other manageable physical activity can improve several of the risks travelling with abdominal fat.
Should you Get Your Vitamin D Tested?
Not necessarily. Australian guidance does not recommend routine vitamin D screening for every healthy adult. Testing is generally reserved for people with recognised risk factors or relevant medical conditions, including osteoporosis, malabsorption, deeply pigmented skin, chronic severe lack of sun exposure, certain medications and kidney disease.
Obesity is associated with lower vitamin D, but a large waist by itself is not a reason to march into your GP’s office demanding a test. It is a reason to discuss your overall risk, particularly if other risk factors apply.
And do not megadose vitamin D on the strength of this study. An Australian randomised trial involving 21,315 adults aged 60–84 found that monthly vitamin D supplementation did not reduce overall mortality in an unscreened population.
That does not mean confirmed deficiency should be ignored. It means treatment should be based on your circumstances and professional advice, not a frightening headline and the largest bottle available at Chemist Warehouse.
Sun exposure is not a licence to bake yourself either. The Cancer Council recommends sun protection whenever the UV Index reaches three or above. In Queensland, that is much of the year.
The practical conclusion is less dramatic than the headline but considerably more useful: measure your waist, work on reducing it if necessary, and speak to your GP about vitamin D testing if you have genuine risk factors. This study identified a warning sign. It did not discover a vitamin pill that makes belly fat harmless.
